Enrollment forms
ALL forms must be returned in order to be considered for enrollment.
Information will be forwarded once your application has been reviewed. 
A non-refundable $100 deposit is required for your application to be reviewed. 
 

There are 3 ways to submit your application:

• Email to amy.brewer@mcpsmd.net

• Mail to PBHS, Child Development, Amy Brewer, 14121 Old Columbia Pike, Burtonsville, MD 20866

• Drop it off at the school in the main office

• Your deposit should be mailed or dropped off. Check or money order only .
